Discussed provider requesting zoning documents from Fairfax County to increase capacity.
An unannounced monitoring inspection was conducted today. Observed 4 children (12-points) directly supervised by 2 caregivers. Ratios and supervision were in compliance. A sample of 3 children's records, and 2 caregiver records were reviewed. The physical plant, and emergency procedures were also reviewed. No medications were reported on site. The children were engaged in various activities, to include: free play, snack time, rest time, as well as potty training and hand washing routines. The interior of the family day home was found to be clean, organized, and well supplied with toys and equipment for the children. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-790 Description: Based upon observation and interview, required posted telephone numbers were not posted. Evidence: The local dial number for police, telephone number for responsible person for back up care and regional poison control telephone number were not posted. Plan of Correction: Provider will post required numbers on a board next to door way.
Standard #: 22VAC40-111-800-A Description: Based upon record review, required documentation was found to be missing or incomplete. Evidence: The provider did not have a completed emergency response plan with required evacuation locations and shelter in place procedures. Plan of Correction: Inspector emailed provider model forms during inspection to use for emergency plan.
Standard #: 22VAC40-111-940-B Description: Based upon observation, a thermometer was not available to monitor the refrigerator and freezer temperatures as required. Plan of Correction: Provider will purchase thermometer.
Standard #: 22VAC40-111-960-E Description: Based upon observation and interview, bottles and food for infants in care were not labeled with individual child's name and date. Plan of Correction: Provider will label bottles and food from home when more than one child is in care.
Standard #: 22VAC40-111-60-B Description: Based upon record review, required information was missing or incomplete. Evidence: The record for child A was missing addresses for emergency contacts #1 and #2. Plan of Correction: Provider will ask parent for missing information at pick up time.
Standard #: 22VAC40-111-280 Description: Based upon observation, cleaning agents were not kept in a place inaccessible to children in care. Evidence: The cabinet containing cleaning supplies in the bathroom used by children did not have a child safety lock to keep products inaccessible as required. Plan of Correction: Provider will put a cabinet lock on the cabinet in the bathroom.
