Comments:

A /renewal inspection was conducted today between 9:00am and 10:30am. There were 43 children present, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 7 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22.1-289.031-A
Description:	Based on observation, the child day center did not post the fact that it is exempt from licensure in a visible location on the premises. Evidence The child day center did not have the notice of exempt from licensure posted.
Plan of Correction:	I didn't know it need to be posted. Notice will be posted today.
Standard #:	22.1-289.035-B-2
Description:	Based on review of staff records, there is not documentation of submission to fingerprinting for all staff. Evidence Staff #1, first day of employment 4/23/2023, does not have documentation of submission to fingerprinting. There is no eligibility letter in their record.
Plan of Correction:	Staff will make an appointment and have the fingerprint completed. I will send the inspector the results when received.
Standard #:	8VAC20-770-40-D-2
Description:	Based on review of staff records, the center does not have documentary evidence that the center is in compliance of submitting a search of central registry within 30 days of the first day of employment. Evidence Child #2, first day of employment 8/24/2018, does not have documentation of a central registry search.
Plan of Correction:	The CPS form was sent to the staff member instead of the center, she has already completed the new form. I will submit the form today. I will send the results to the inspector when received.
