Technical Assistance:
We discussed the requirements for children's physical examination reports. The risk assessment for a violation was provided. We talked about your written plan and notice to parents to meet the requirements for the notice from the Division of Licensing. Background checks are due before three years from the date of the last record. You were encouraged to review the requirements as a subsidy vendor.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 12:05 PM to 3:45 PM. Today, there were ten children in care, ages infancy to fourteen years (seventeen points), and one resident child with two caregivers. I viewed daily routines, activities, nap, staffing, supervision, interactions with children, nutrition/menu, posted information, diapering procedures, equipment, five records for children, three caregivers/resident records, caregiver training, emergency supplies and drill records, attendance records, indoor and outdoor areas. The play equipment under the trees has not been in use during day care hours. We talked about the opportunities for outdoor play. Today, the children enjoyed a variety of toys. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of five records for children, and an interview with the provider, the physical examination for child 5 was obtained late and not within the first thirty days of attendance. Evidence: Child 5 started attending on March 21, 2017 and only the immunization report was obtained prior to this date. The physical examination report was signed by the health care provider on June 6, 2017.

Plan of Correction: The provider will speak to parents of children who are new and make sure the physical exam reports are provided within the first thirty days of attendance. The paperwork provided by parents when children enroll will be viewed to determine what is needed from the parents on time.
