The following inspection took place between the hours of 10:00 am through 11:00. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced renewal inspection there were a total of 7 children (11 points) with 2 caregivers. A total of 3 children's files and 1 staff's file were reviewed. The children were engaged in the following: toy play, games, music and movement. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-180-A Description: Based on record review, a caregiver does not have a current TB screening. Evidence: Caregiver #1 does not have a current TB screening, the last TB screening is dated 6/23/2015. Plan of Correction: The caregiver will provide a copy of her recent TB for the FDH.
