Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, July 11, 2017, from approximately 9:45am until 11:15am. Present today were six children present totaling fifteen points with the provider. The children were using the rice sensory table, looking at books and identifying colors. Practice Evacuation Drill: 6/19/17 Morning Snack: cereal and bananas Lunch: Lasagna, sliced grapes, peas and milk Three children's records were inspected. The provider's record was inspected.

Violations:
Standard #: 22VAC40-111-140-B-3
Description: Based on review of the provider's record, the provider did not have current certification in cardiopulmonary resuscitation (CPR). Evidence: The provider's CPR expired 2/23/17.

Plan of Correction: Per Provider: I will register for a CPR class as soon as possible.

Standard #: 22VAC40-111-140-B-4
Description: Based on review of the provider's record, the provider did not have current certification in first aid. Evidence: The provider's first aid certification expired 2/23/17.

Plan of Correction: Per Provider: I will register for a first aid class as soon as possible.

Standard #: 63.2-1809-A
Description: Based on inspection of children's records, the center did not ensure each record contained documentation of proof of identity. Evidence: Out of three children's records inspected, Child #1 did not contain information regarding proof of the child's identity and age.

Plan of Correction: Per Provider: I have requested this information from the family.
