Technical Assistance:
The laundry room by the bathroom used by the children was discussed with the provider. The bathroom area is gated with a 2 - 3 foot high gate. The provider must ensure that, as the children grow taller, a child cannot climb over this gate. When the door to the laundry room is closed, there is a child proof knob cover in use.

An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:15 AM and departed at 2:10 PM. Ten children, totaling 18 points were in care with two caregivers. Swimming activities, morning activities, food service and nap time were observed. Basic water safety certification was verified for two caregivers. The provider stated that no medication is administered to children in care and no children's injuries were reported. A sample size of three caregiver records and five children's records were reviewed. An exit meeting was conducted with the provider prior to closure of the inspection. Standard #: 22VAC40-111-90-A
Description: Based upon review of five children's records and provider interview, the provider has not ensured that documentation verifying that the child has been adequately immunized is obtained before the child can attend the family day home. Evidence: 1. The record provided for child 5 did not include documentation of that the child is adequately immunized. The child was in care during the inspection. 2. The provider was unable to locate documentation of child 5's immunizations.

Plan of Correction: The provider responded with the following: The parent was contacted during the inspection and brought a second copy of the immunization documentation that is now attached to the child's file making the file complete.

Standard #: 22VAC40-111-240-A
Description: Based upon observation, the provider has not ensured that areas and furnishings of the family day home, inside and outside, are maintained to be safe. Evidence: There is a white Little Tikes climbing structure with a slide and a tunnel. The tunnel is cracked along the top, creating a pinch point with sharp edges.

Plan of Correction: The provider responded with the following: The tunnel was removed from the climbing structure prior to closure of the inspection. All remaining equipment are in great condition.
