Today's visit was a routine monitoring inspection. The inspection began at 9:40am and concluded at 1:15pm. There were seven school age children present and a total of six points. During the inspection children's activities to include games were observed, the outdoor play area was viewed, emergency supplies were viewed, four out of eleven children's records were reviewed, the licensee/vendor's record was reviewed, the assistant/adult household members record was reviewed, lunch time was observed, and some policies and procedures were reviewed to include the emergency preparedness plan. The provider stated she does not provide transportation, the program does not administer medication, and there are only two adult household members residing in the home. There were no children enrolled with diagnosed food allergies at the present time. Discussion took place regarding food allergy requirements for subsidy health and safety requirements. Standard #: 22VAC40-111-70-A
Description: Based on the inspector's review of four out of eleven children's records, the record for one child does not include evidence that the parent(s) have been notified of the family day home policy for the administration of medication. Evidence: The record for child #3 does not include evidence that the parent(s) have been made aware of the of the family day home's policy to administer medication. The child was present during the day of the inspection.

Plan of Correction: The provider will have the parent complete the medication policy form at the end of the program day upon pick up.

Standard #: 22VAC40-111-210-A
Description: Based on the inspector's review of the licensee's and assistant's record, the provider failed to ensure the assistant had at least sixteen hours of training in the 2016 annual year. Evidence: The record for the assistant had six hours of documented training for the 2016 annual year. The standard currently requires sixteen hours of annual training for existing assistants.

Plan of Correction: The licensee will ensure the assistant achieves at least sixteen hours of annual training and will document it in the record.
