Technical Assistance:
Annually, a walkthrough of the home must be conducted and all areas of the home must be observed. Please ensure that the licensing inspector has access to all rooms in the home at least once annually. "Sanitized" means treated in such a way as to remove bacteria and viruses from inanimate surfaces through first cleaning and secondly using a solution of one tablespoon of bleach mixed with one gallon of water and prepared fresh daily or using a sanitizing solution approved by the U.S. Environmental Protection Agency. The surface of the item is sprayed or dipped into the sanitizing solution and then allowed to air dry.

An unannounced renewal inspection was conducted beginning at 10:50 a.m. to 12:45 p.m. with the provider and assistant. There were seven children in care ranging in age from sixteen months to ten years of age for a total of fourteen points. Children were observed watching an age appropriate television program. After television time, children had free play and played circle time with the ten year old. Eight children's records and three staff records were reviewed. Emergency drills were reviewed and complete. The menu was posted. Emergency supplies were reviewed. Staff training was reviewed. The provider and assistant are not Medication Administration Training (MAT) certified and cannot administer medication. Standard #: 22VAC40-111-200-A
Description: Based on review of documentation, orientation was not documented. Evidence: Staff #3, longterm assistant, did not have documentation of orientation training.

Plan of Correction: Staff #3 has been on medial leave since 22nd Aug 2016. Provider will ensure staff #3 had complete orientation and had some documented on recovery.

Standard #: 22VAC40-111-210-A
Description: Based on review of documentation, not all staff obtained sixteen hours of training annually. Evidence: Staff #3 did not have any training documented.

Plan of Correction: Provider will ensure staff obtained 16 hrs annual training. Staff #3's training programs shall be documented upon completion.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface was not nonabsorbent. Evidence: The diapering pad had several rips and tears in the nonabsorbent cover.

Plan of Correction: Denied. Diapering surface was non-absorbent. One diapering pad had 2 minor tears only. The diapering pad shall be replaced.
