- Technical Assistance:
Previous violations were discussed and brought into compliance. Discussed record keeping, the future requirements for background checks and requirements for assistants. The provider also showed proof of the purchase of 3 new cribs to be delivered this week and proof of enrollment in training classes. Another assistant is possibly starting soon.
An unannounced renewal study was conducted today from 10:15am to 12:30pm. 6 children (22 points) were directly supervised by 2 caregivers. The physical space, attendance records, children's records, caregiver records, pet records, evacuation drills and the emergency preparedness plan were reviewed. Required documents were posted. No medications were on site. Parents provide the food for children. The space was clean, organized and contained an abundance of age appropriate materials. Standard #: 22VAC40-111-960-E Description: Based on observation, refrigerated bottles shall be labeled with the child's full name and the date, if more than one infant is in care. Evidence: 4 out of 4 bottles inspected were not labeled with dates. Plan of Correction: We will make sure they are labeled correctly.
Standard #: 22VAC40-111-120-B Description: Based on record review, the provider did not obtain the required information or maintain a record for Assistant #1. Evidence: The file for Assistant #1 was incomplete. The following was missing: personal information, emergency contacts, 2 reference checks, background checks and documentation of education and training. Assistant #1 only had the Sworn Disclosure statement, TB screening and CPR/First Aid Certificate available for review. The assistant started employment on August 9, 2017. Plan of Correction: The background checks will be sent off today and I will complete the rest of the information.
Standard #: 22VAC40-111-60-B Description: Based on record review, the provider did maintain children's records to contain the required information. Evidence: 3 out of 6 records did not contain the required information. The Proof of ID documentation was incomplete for Child #1, the Liability Insurance form was not signed for Child #2 and Child #3 had immunization records but no health examination as required by 22 VAC 40-111-100. Plan of Correction: I will get all of the information today.
Standard #: 22VAC40-111-130 Description: Based on record review, the provider did not meet the requirements specified in 22 VAC 40-191. Evidence: The subsequent Criminal History check was not available for review. The latest record was dated 3/15/2011. Plan of Correction: I must have misplaced it or accidently thrown it away. I will get a new one right away.
Standard #: 22VAC40-111-200-A Description: Based on record review, the provider did not orient the assistant by the end of their first week of assuming job responsibilities. Evidence: Assistant #1 began employment August 9, 2017 and as of today did not have documentation of orientation. Plan of Correction: I will finish her file today.
Standard #: 22VAC40-111-320-B Description: Based on observation, the provider did not have outlet covers installed on all unused electrical outlets accessible to children under the age of five years of age. Evidence: 2 outlets were uncovered in a room where all children were under the age of 5. Plan of Correction: I will cover those right away.
