An unannounced renewal inspection was conducted with provider and assistant. There were 11 children present, ranging in age from 3 months to 4 years old, totalling 31 points. The children were waking from rest and having afternoon snack. Snack served today: crackers and boiled egg. Two children's files were reviewed - please see Supplemental Summary for missing item. Household/staff files reviewed are complete. There are no children receiving medication at day care today - provider is certified in Medication Administration Training until 1/25/2018. Standard #: 22VAC40-111-100-A Description: Based on review of two children's files, the provider failed to have documentation of a physical within 30 days of each child's first attendance. Evidence: Child B (enrollment date 2/2017) did not have documentation of a physical. Plan of Correction: Provider will request the physical from the parents.
