Comments:

A code compliance inspection was initiated on Wednesday, May 23, 2023 and concluded on Thursday, May 25 2023. On May 23, the inspector was on site from 3:40pm to approximately 4:45pm. There were 32 children in attendance with four staff directly supervising. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. The center is equipped with toys and supplies and items were available to the children. All areas of the facility including classrooms, hallways, the outdoor play area, and bathrooms were inspected. Five children's records and four staff records were reviewed. Additional documentation was submitted electronically on May 25, 2023. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description:	Based on a review of four staff records and interview, the center did not ensure four staff had the satisfactory results of the fingerprint-based national criminal background check prior to employment. Evidence: 1) The records of Staff #1, Staff #2, Staff #3, and Staff #4 did not contain the results of the fingerprint-based national criminal background check. 2) During interview, a member of management acknowledged the fingerprint-based national criminal background checks for Staff #1, Staff #2, Staff #3, and Staff #4 have not been obtained to date. The center received the initial religious exemption status letter on 03/16/2023. The staff were considered employees at the time of approval and the fingerprints should have been obtained prior to 03/16/2023.
Plan of Correction:	The background checks will be requested. In the future, staff will obtain fingerprints prior to employment.

Description:	Based on a review of four staff records and interview, the center did not ensure four staff had the required background checks within the required timeframe. Evidence: 1) The records of Staff #1, Staff #2, Staff #3, and Staff #4 did not contain a complete sworn statement or affirmation or a search of the central registry. 2) During interview, a member of management confirmed Staff #1, Staff #2, Staff #3, and Staff #4 did not complete a sworn statement upon receipt of the initial religious exemption status letter dated 03/16/23. A search of the central registry was not requested for the staff members within 30 days of approval. The staff were considered employees at the time of approval and a sworn statement should have been completed before employment or commencement of service. A search of the central registry should have been obtained within 30 days of employment or commencement of service.
Plan of Correction:	Staff have completed the sworn statements and the central registry searches will be submitted. In the future, the background checks will be obtained within the required timeframe.
