An unannounced monitoring inspection was initiated today at 1:50pm. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 8 children, totaling 17 points, under the supervision of 2 caregivers. Today, the children made ice cream using plastic bags (vanilla, chocolate and caramel), picked tomatoes and rested during nap time. Today, the children were served chicken vegetable soup, tomatoes, rolls and milk. The Provider is not certified in MAT (medication administration training)and there are no children with any medication needs. Both caregivers are certified in CPR and First Aid. Standard #: 22VAC40-111-90-C Description: Based on file review, 2 out of 5 children's records did not have documentation of additional immunizations once every six months up to the age of 2 years. Evidence 1. Child #1 is 28 months old. The most recent immunization record on file was completed when the child was 15 months old. 2. Child #2 is 27 months old. The most recent immunization record on file was completed when the child was 20 months old. Plan of Correction: The Provider texted the parents reminding them to bring this in to the FDH during today's inspection.
