63.2 Liability Insurance Disclosure
Today an unannounced monitoring visit was conducted with Provider, this visit took place in the morning between 9:30-11:45 There were 2 kids present for a total of 8 points within the supervision guidelines. The children were observed having morning snack, free play, coloring, singing abc songs, lunch. Morning snack consisted of grapes and crackers. Lunch consisted of quinoa, vegetables, chicken and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu, attendance record and emergency procedures. Standard #: 22VAC40-111-920 Description: Based on observation, the menu posted did not reflect what was being served. Evidence:Menu posted for am snack was goldfish and strawberries however eggs, and grapes were served. Plan of Correction: Provider stated that she haven't been to the store and will make a change to the menu to reflect what she is serving today.
Standard #: 22VAC40-111-60-B Description: Based on review of the children records, one record did not include two emergency contact persons name, address and phone number,parents work address and number. Evidence: Child #1's record, only had one emergency contact person listed, the fathers employers phone number was missing. Plan of Correction: Provider will ask for the information today.
Standard #: 22VAC40-111-680-B Description: Based on observation, caregiver did not wash a child's hands with soap and running water. Evidence: Provider was observed giving snack to a child, however the child's hands were not washed with soap and water. Plan of Correction: Provider stated that she had a wet paper towel. Provider will wash children hands with soap and water at all times.
