Consultation provided on the repeat violation of using a pressure mounted gate at the opening of the stairs. Please do a shelter in place drill by the end of September. Make sure you keep the findings from the most recent inspection posted.
An unannounced monitoring inspection was conducted today with the provider who was present with three toddlers ( 16, 17 and 20 months of age) for a total of 9 points. The provider does not administer medication. Three children and household member's files were reviewed. There have been no injuries/accidents documented. Arrival time: 3:25 pm Departure time: 5: 25 pm
Standard #: 22VAC40-111-100-A Description: Based on a review of three children's files, it was determined that one child did not have a physical on file within 30 days of enrollment. Evidence: Child A enrolled on 5/29/17 and there was no physical on file. Plan of Correction: I will get the physical.
Standard #: 22VAC40-111-110-B Description: Based o a review of children's files, it was determined that the immunization record for one child was not signed by a physician/designee. Evidence: The immunization record for child A was not signed. Plan of Correction: I will get a signed copy.
Standard #: 22VAC40-111-170-A Description: Based on a review of three household member's files it was determined that two adult household members did not have tuberculosis test/screening on file. Evidence: Person A and B turned 18 years of age on 3/29/2017 and neither have a tuberculosis test/screening on file Plan of Correction: I will get them a tuberculosis test or screening.
Standard #: 22VAC40-111-430-B Description: Based on an inspection of the upstairs area, there was a pressure mounted gate at the top of the stairs opening. Evidence: There was a pressure mounted gate at the top of the stairs opening while 16, 17 and 20 month old toddlers were present in the upstairs area. Plan of Correction: When the door bell rang I took the gate which was being used in an area to prevent the children from reaching the stairs and placed it at the opening in front of the stairs. I will make sure the pressure mounted gate is not placed or used at the stairs opening.
Standard #: 22VAC40-191-40-D-1-c Description: Based on a review of household member's files, it was determined that one file did not have a repeat criminal history report check on file every 3 years. Evidence: The criminal history report check for Person C was dated 7/25/11. Plan of Correction: I need to locate my most recent criminal history report.
Standard #: 22VAC40-191-40-D-4-a Description: Based on a review of files, it was determined that after turning 18 years of age, two household member's have not obtained a criminal record report within 30 days. Evidence: Person A and B turned 18 years of age on 3/29/17 and to date, they do not have criminal record reports on file. Plan of Correction: I am in the process of sending off for these background checks.
