Discussion of the subsidy regulations and how they apply to the Family Day Home (FDH). It was also discussed that the Provider has returned her "scanner" to DSS back in December 2016. It is her hope to obtain subsidy again in the near future.
A Subsidy Health and Safety (SHSI) inspection was conducted at the Luz De Gallo FDH located in Ashburn, Virginia. This inspection was conducted on 09/06/2017, between the hours of 2 pm through 4:30 pm to review supplemental health and safety requirements. An Unannounced Renewal licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. If you should have any future questions, please call. Standard #: VENDHOM-000-012-C Description: Based on interview, 2 of 3 staff/household member's files reviewed indicated that all caregivers hired prior to the effective date of this regulation, did not complete Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services. Evidence - 1. Member #1 stated (during an interview conducted on the date of inspection - 09/06/2017) that neither her nor Member #2 had complete the 10 hours of online Pre-service training. 2. Also a certificate was not available for either Member #1 or Member #2. Plan of Correction: Training will be looked into and obtained by both Member #1 and Member #2.
Standard #: VENDHOM-000-012-E-1 Description: Based on review, 1 of 3 staff/household member's files reviewed indicated that a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care was not available. Evidence - 1. On the date of inspection (09/06/2017), the most current CPR certification for Member #2 was dated to expire on 09/05/2017. 2. This was confirmed on the date of inspection by the Provider. Plan of Correction: Member #2 is scheduled to recertify in CPR on 09/09/2017.
Standard #: VENDHOM-000-012-E-2 Description: Based on review, 1 of 3 staff/household member's files reviewed indicated that a current certification in first aid was not available. Evidence - 1. On the date of inspection (09/06/2017), the most current First Aid certification for Member #2 was dated to expire on 09/05/2017. 2. This was confirmed on the date of inspection by the Provider. Plan of Correction: Member #2 is scheduled to recertify in First Aid on 09/09/2017.
Standard #: VENDHOM-000-029-B-5 Description: Based on review, the Family Day Home (FDH) Emergency Preparedness and Response Plan (EP&RP) did not include Lockdown procedures. Evidence - 1. On the date of inspection (09/06/2017), the FDH's EP&RP posted on the parent bulletin board did not include procedures with regards to Lockdown. 2. The was confirmed on the date of inspection by the Provider. Plan of Correction: Lockdown procedures shall be added to the FDH EP&RP and practices at least once a year.
