Technical Assistance:
During the inspection, the licensing inspector discussed with the provider emergency plans, diapering and behavioral guidance.

The following inspection took place between the hours of 11:15 am through 1:40. At the start of this unannounced monitoring inspection there were a total of 3 children (6 points) with 1 caregiver. A total of 3 children's files and 3 staff/household member's files were reviewed. Licensing inspector observed children engaged in toy play, games, lunch, naptime and other organized activities. Standard #: 22VAC40-111-730-D
Description: Based on observation, provider did not ensure that medications were kept in a locked place to prevent access to children. Evidence: Two bottles of over the counter medications were observed on a shelf in the kitchen where children eat their lunch.

Plan of Correction: Corrected during inspection. For the long term, we will replace the current lock on storage for medications with something more secure.

Standard #: 22VAC40-111-750-A
Description: Based on review of records, provider did not obtain parent authorizations for topical skin products. Evidence: Child A, B and C's files did not contain parent authorizations for provider to administer diaper ointments.

Plan of Correction: I will have the parents sign the form.

Standard #: 22VAC40-111-280
Description: Based on observation, provider did not ensure that potentially poisonous substances were inaccessible to children. Evidence: 1. Hand sanitizer and nail polish remover were observed on a dining room table that was accessible to children. 2. Disinfectant wipes were located on a kitchen counter next to a table where the children ate their lunch. 3. Hand sanitizer was observed on an end table in the living room where children nap and play.

Description: Based on observation, the provider did not ensure that flammable materials were inaccessible to children. Evidence: 1. An open flame was located on top of a shelf within 3 feet of reach of children. 2. 3 boxes of matches were observed in an unlocked cabinet where children play.

Standard #: 22VAC40-111-530-H
Description: Based on observation, provider did not ensure that pillows were assigned and covered with pillowcases. Evidence: During naptime, children's pillows were not identified for individual use or covered with pillowcases.

Plan of Correction: I will get them this evening.
