Technical Assistance:
Discussed with provider: annual updates to children's files, recommendations.

Comments:
The inspection took place between 9:00 am through 10:45 am. At the start of this unannounced monitoring inspection there were a total of 10 children (30 points) with 3 caregivers. A total of 6 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, games, morning snack, bathroom breaks, hand washing, and outside time. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, smoke detector was not maintained in an operable manner. Evidence: Smoke detector at the stairs, entrance to the daycare, was chirping indicating the battery needs to be replaced.

Plan of Correction: I will replace the battery in the smoke detector.

Standard #: 22VAC40-111-320-A
Description: Based on observation, a small appliance was not unplugged while not in use. Evidence: A toaster was observed plugged in, in the kitchen.

Plan of Correction: It was unplugged during the inspection.

Standard #: 22VAC40-111-330-A
Description: Based on provider statement, there is no land line telephone available.

Plan of Correction: I will get a new phone today.

Standard #: 22VAC40-191-40-D-1-a
Description: Based upon provider statement, a new person requiring a Central registry search (CPS) does not have a completed search on file. Evidence: Adult A - Date of Birth 2/8/2002, did obtain a CPS search upon turning 14 years of age.

Plan of Correction: I will submit the search today.
