Comments:
An unannounced renewal inspection was conducted today from 12:10pm-1:45pm. There were 11 children (33 points) directly supervised by 3 staff and one visitor. The home, 4 staff records, 4 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, preparing for and taking a nap. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-710
Description: Based on review, the provdier failed to ensure that children's medication had documentation of an authorization form. Evidence: Reviewed 4 children's records and found that CH4 did not have documentation of medication authorization on file.

Plan of Correction: Will have parents complete today.

Standard #: 22VAC40-111-830-A
Description: Based on review, the provider failed to ensure that the emergency evacuation procedure was practiced monthly. Evidence: Reviewed the evacuation and shelter-in-place drill log and found that in February and In August, the provider failed to practice a monthly emergency evacuation drill, but did practice shelter-in-place drills those months.

Plan of Correction: I will do it.

Standard #: 22VAC40-111-100-A
Description: Based on review, the provider failed to ensure that they obtained documentation of a physical examination within 30 days after the first day of attendance. Evidence: Reviewed 4 children's records and found that CH3 did not have documentation of a physical examination.

Plan of Correction: Parent will bring today.

Standard #: 22VAC40-111-120-B
Description: Based on review, the provider failed to ensure that the assistants records contained the required information. Evidence: Reviewed 3 staff records and found that Staff 1 did not have documentation of 2 reference checks on file. Evidences, this was previously cited on 7/29/2016.

Plan of Correction: I will have it corrected today.

Standard #: 22VAC40-111-320-A
Description: Based on observation, the provider failed to ensure that small electrical appliances such as, but not limited to, curling irons, toasters, blenders, can openers and irons shall by unplugged unless being used by the caregiver or with children under close sueprvision. Evidence: Observed that the electric kettle and toaster were plugged in on the main level on the home and not in use.

Plan of Correction: Corrected during the inspection.
