Technical Assistance:
Reviewed all current and upcoming changes to background check procedures. All future communication regarding background checks will only be distributed via email.

An unannounced monitoring inspection was conducted today. Children's records, caregiver records, the indoor and outdoor physical plant, and emergency preparedness items were inspected today. The provider reported that there are no children's medications on site at this time. The children were observed eating snack, finishing lunch, and preparing for their afternoon nap. The children had been served chicken, lentils, rice, mixed vegetables, and milk for lunch today. This inspection was completed between approximately 11:00am and 12:30 pm. Standard #: 22VAC40-111-800-B
Description: Based on a review of emergency preparedness documents, it was determined that the provider has not conducted an annual review of the emergency preparedness plan. Evidence: The provider most recently reviewed the emergency preparedness plan on 9/8/16.

Plan of Correction: I will review the plan this weekend.

Standard #: 22VAC40-111-900-A
Description: Based on observations, it was determined that the provider did not serve food in a manner to help prevent choking. Evidence: The Inspector observed two children under the age of 4 years eating grapes for snack. The provider did not cut the grapes in order to help prevent choking. One child was age 2, the other child was age 3.

Plan of Correction: The grapes were cut during the inspection.

Standard #: 22VAC40-111-920
Description: Based on observations, it was determined that the provider has not included all required information on the posted menu. Evidence: The posted menu does not include the date.

Plan of Correction: I will date the menu each week.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record #1 and #2 do not include documentation showing that each file has been reviewed by the parent within the past year. Each parent did not include the full date of when they completed the registration form. The Inspector was unable to verify when the parent most recently reviewed the information.

Plan of Correction: I will ask each parent to fully document the date.

Standard #: 22VAC40-111-320-B
Description: Based on observations, it was determined that one electrical outlet was not covered. Evidence: One outlet in the basement play area was not covered.

Plan of Correction: I will cover the outlet today.

Standard #: 22VAC40-111-690-D
Description: Based on observations, it was determined that the diaper changing surface is not a non-absorbent surface. Evidence: The provider has placed a cotton cloth on top of the diaper changing pad. This cloth is not non-absorbent.

Plan of Correction: The cloth cover was removed during the inspection.

Standard #: 22VAC40-111-690-E
Description: Based on observations, it was determined that soiled diapers are not disposed of in a trash can that is properly lined. Evidence: There is no liner inside the diaper trash can.

Plan of Correction: I will put the trash bag inside the diaper trash can.
