An unannounced Code Compliance inspection was conducted with the acting director on 5/12/23 from 10:00 - 11:15 am. There were 72 children present, supervised by 14 staff. Children were observed engaged in circle time activities, playing in centers, eating snack and outside on a scavenger hunt. Disclosure documents were reviewed along with four staff and four child records. Evidence: the record for staff #1, staff #2, staff #3 and staff #4 did not contain documentation of a sworn statement or Central Registry Search results.
