A Subsidy Health and Safety inspection was conducted on October 13, 2017 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDHOM-000-005-B Description: Based on record review, the vendor provider's written hard copy record of attendance does not document departure. Evidence: 1. Attendance record reviewed and has names of children present on each day but does not record departure. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: VENDHOM-000-029-A Description: Based on review of vendor provider's emergency preparedness plan, it was determined the plan does not include lockdown procedures. Evidence: 1. Inspector reviewed the 4 page plan but the plan did not include how she would address and handle a lockdown situation. Plan of Correction: I am attending a Emergency Preparedness training next Saturday and will have it completed with the help of local emergency personnel.
