A Subsidy Health and Safety inspection was conducted on 10/12/17 to review supplemental health and safety requirements. n unannounced monitoring inspection was completed on 10/12/17. There were four children in care at arrival of the inspector and two more children arrived at approximately 9:30 am. The point total for the six children was 16. The provider was the caregiver providing the care and supervision of the children. Six children's records (two newly enrolled children) were reviewed, the provider's record was reviewed, and a newly hired assistant caregiver's record was reviewed. No medications at the family day home. The inspector arrived at the provider's home at 9:00 am and departed at 1:00 pm. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDHOM-000-012-B Description: Based on record review and interview, the provider failed to ensure that within the first 90 days of employment or service all caregivers completed the Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services. Evidence: 1. An assistant caregiver who was employed as of 5/26/17 had not completed the required ten hour VA Pre-Service training on line. There was documentation that the assistant had worked at the family day home 22 days during July and August 2017. According to the provider, the assistant is only used during the summer and during college breaks. Plan of Correction: The provider will make sure that the assistant completes the required training prior to working at the FDH at the next college break.
