- Technical Assistance:
Discussed using a nonabsorbent surface on the changing table, the cloth was removed. Spray with a bleach and water solution. Reviewed the use of the emergency drill form to practice evacuation and shelter drills. Reviewed again the child record forms. Reminder to keep diaper creams and ointment away from the children, place in a n area that are not accessible to children. When there is more than one child with a bottle, each bottle needs to be labeled with the child's name and dated. Reminded that the violation notice and license needs to be posted.
An unannounced monitoring inspection was conducted today between the hours of 8:00 am through 10:00. There were a total of 1 children (4 points) with 1 caregiver. A total of 2 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, bottle, diapering and nap. Standard #: 22VAC40-111-750-A Description: Based on record review and observation, the provider has not obtained written parent authorization noting any known adverse reactions for child's diaper ointment. Evidence: Child #1 had diaper ointment at the family day home, the provider has not obtained written authorization to apply the diaper ointment. Plan of Correction: I will get the parent to complete a form for the ointment.
Standard #: 22VAC40-111-830-A Description: Based on record review and provider interview, the emergency evacuation procedures have not been practiced monthly with all caregivers and children in care during all shifts that children are in care. Evidence: Child #1 and child #2 started 9/5/17 and 9/6/17. The provider did not practice an emergency evacuation the month of September. Plan of Correction: I will do a fire drill this month.
Standard #: 22VAC40-111-60-B Description: Based on record review, the provider has not maintained a complete record for each child. Evidence: Two children records were reviewed, the following information was not maintained in the child's record; Child #1 and #2 record does not have 2 complete addresses for emergency contact. Child #2 record does not have the medical insurance policy number. Child #1 and #2 does not have a completed proof of identity information. Child #1 and #2 does not have a completed physical examination, children first day of attendance was 9/5/17 and 9/6/17. Plan of Correction: I will ask the parent today for this information.
Standard #: 22VAC40-111-70-A Description: Based on record review, the provider has not provided to the parents before the child's first day of attendance the required information. Evidence: Child #1 first day of attendance was 9/5/17 and child #2 first day of attendance was 9/6/17. Child #1 and #2 records do not have the model form "Information for Parents" form completed. Plan of Correction: I will give the parents the Information for Parents form today.
Standard #: 22VAC40-111-70-B Description: Based on record review, the provider has not obtain the parent's written acknowledgement of the receipt of required information. Evidence: Child #1 and Child #2 records do not have signed written acknowledgement for Information for Parents form, Policy for the administration of medications, Liability Insurance Declaration, and the Provisions of the Emergency Preparedness and Response Plan. Plan of Correction: I will get the written authorization from the parents.
Standard #: 22VAC40-80-120-E-1 Description: Based on observation and provider interview, the most recently issued license has not been posted at each public entrance of the facility. Evidence: The license has not been posted, the provider stated that she has not posted the license. Plan of Correction: I will get it posted today.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation and provider interview, the findings of the most recent inspection of the facility have not been posted. Evidence: The most recent violation notice dated 7/26/2017 was not posted. The provider stated that she has not posted the notice . Plan of Correction: I will post it today.
