Today an unannounced Monitoring inspection was conducted with Lisa Hoang. This visit took place in the late morning between 10:50am and 12:40pm. There were 9 children present with the provider and 1 assistant for a total of 22 points within the supervision guidelines. The children were observed eating lunch, dancing, playing with farm animals and getting ready for nap. Positive interactions with the children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all of the required information. Evidence: Child #1 file was missing their medical insurance policy number and a physical from the child's physician. Child #2 file was missing their medical insurance company name, medical insurance policy number, proof of the child's age and identity and a physical from the child's physician. Child #5 file was missing the phone number for their second emergency contact. Plan of Correction: I will have all of them taken care of.
Standard #: 22VAC40-111-280 Description: Based on observation, poisonous substances were not stored in an area inaccessible to children. Evidence: There are various cleaning materials and disinfectants stored under the sink in the kitchen. The child-lock that is on the cabinet door does not work. Plan of Correction: That will be easy for my husband to fix. He can put a new lock on.
Standard #: 22VAC40-111-320-B Description: Based on observation, child-resistant protective covers were missing from unused electrical outlets. Evidence: One outlet cover in the Toddler play area was missing a protective cover. Plan of Correction: This was fixed during the inspection.
