Technical Assistance:
During the inspection, licensing representative discussed with provider updating medication administration policies, parents signing allergy information page, and model forms on the VA DSS website.

Comments:
The following inspection took place between the hours of 10:45 am through 1:00pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 9 children (33 points) with 3 caregivers. A total of 2 children's files and 3 staff/household member's files were reviewed. The children were observed actively engaged in a variety of activities including, playing with age appropriate toys and having lunch of vegetable soup. Standard #: 22VAC40-111-800-B
Description: Based on review of emergency preparedness procedures, the emergency plan was not updated annually. Evidence: The emergency plan on file was last reviewed May 23, 2016.

Plan of Correction: Updated during inspection.

Standard #: 22VAC40-111-200-A
Description: Based on a sample review of records, two caregivers did not receive the required training. Evidence: Caregiver #2 and #3 were missing documentation of orientation training on facility policies and procedures. [Caregiver #2 date of hire 6/09/2017.]

Plan of Correction: I will complete the orientation form.

Standard #: 22VAC40-111-210-A
Description: Based on review of record, 1 caregiver did not complete the required 16 hours of annual training. Evidence: Caregiver #3 did not have documentation of completing annual training [from October 2016 to October 2017].

Plan of Correction: I will have the caregiver complete it online.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the findings from the most recent inspection of the facility. Evidence: The notice from the last inspection on 4/12/2017 was not posted.
