An unannounced monitoring inspection was conducted beginning at 1:30 and ending at 3:15. There were 7 children in care including the provider's own child. Children were observed getting settled down for nap and sleeping. Reviewed children's files - see violation notice Reviewed staff and family files - see violation notice Emergency escape drills are up to date as is attendance record. Standard #: 22VAC40-111-960-E
Description: Based on observation, it was determined that there were two infants who had bottles for formula . Evidence: The bottles were not labeled with each child's name and today's date.

Plan of Correction: Bottles will be labeled every day.

Standard #: 22VAC40-111-120-B
Description: Based on a review of records and interview with the provider, it was determine that the assistant who began working on 9/28/17 did not have a complete file. Evidence: Staff A does not have a Sworn Statement or Affirmation that is to be done as of the first day of employment. Also does not have a TB test or screening which was due within 21 days from the start of employment. In addition, the provider failed to complete Staff Orientation for the staff person by the end of the first day of employment. Provider has not completed reference checks for new assistant

Plan of Correction: The provider will have all required information on file.

Standard #: 22VAC40-111-60-B
Description: Based on a review of six children's files, it was determined that 5 files were missing required information. Evidence: Child #1 was missing a signed physical and shot record , and proof of age and identity. Child #2 was missing a Doctor's phone number and Proof of age and identity. Child #3 was missing annual update for information and agreement. Child al update. Child #6 was missing physical and shot record.

Plan of Correction: Provider will check all files and notify parents of missing information.

Standard #: 22VAC40-111-520-A
Description: Based on observation and interview with the provider, it was determined that there are two enrolled infants under 12 months of age and there is only one crib. Evidence : On 10/26 at 1:30 there were two infants in care and one child was in the crib and second child needed to sleep. There was no second crib available.

Plan of Correction: The provider will purchase a second crib this weekend.
