Comments:
An unannounced monitoring inspection was conducted today, 10/30/17. Violations from the previous inspection have been corrected. A violation notice was completed regarding the violations found during today's inspection. 11 children ranging in age from 10 months-4 years totaling 27 points with two caregivers. Reviewed new children and staff/HHM records. Inspection began: 10:05 am ended 1:20 pm

Standard #: 22VAC40-111-520-A
Description: Based on observation and interviews the provider failed to have adequate sleeping arrangements for the children in care. Evidence: 1. Two children under the age of 12 months do not have a crib to sleep in during nap.

Plan of Correction: The two children over 12 months of age will be placed on the cots and the two under 12 months of age will be placed in the cribs.
