Technical Assistance:
Licensing Inspector discussed VAC 40-111-200 and VAC40-191-(BC) with the provider.

An unannounced monitoring inspection was conducted today from approximately 9:20am-10:40am. There were 7 children (20 points) directly supervised by 2caregivers. A sample of 4 children?s and 1 caregiver's records were reviewed. The children were observed eating breakfast, playing with cars and playing with "Mr. Potato Head". One new caregiver has been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-60-A
Description: Based on records reviewed, children's records were found to be incomplete. Evidence: 3 of 3 children's records reviewed were incomplete; 1) Child A was missing the home addresses of both parents, 1 of 2 emergency contacts, and a physical within 30 days, Proof of ID; 2) Child B was missing a physical within 30 days; 3) Child C was missing information for parents.

Plan of Correction: The parents for child #1 fill out the missing information The parent for child #2 will take the child to the doctor to get a physical The parent fill out the missing information

Standard #: 22VAC40-111-340-C
Description: Based on observation, the bathroom did not contain paper towels. Evidence: The bathroom on the main level across from the entrance of the house did not have any paper towels.

Plan of Correction: I will put some in there.

Standard #: 22VAC40-111-600-B
Description: Based on observation, An infant spent over 30 minutes in a confined place. Evidence: Child D (10 months old) was confined in a "Baby Bumbo Floor Seat" from 9:20am-9:55am (35 minutes).

Plan of Correction: I will make sure I don't do that again

Standard #: 22VAC40-111-680-B
Description: Based on observation, Caregiver did not wash the children's hands with liquid soap and warm running water. Evidence: After the children were finished with their breakfast the children's hands were dirty and required hand washing. Caregiver A did not wash the hands of 5 of 5 children.

Plan of Correction: I will make sure wash the hand after a meal
