Technical Assistance:
Discussed the code changes regarding background checks that became effective 7/1/17 which included a new Sworn Statement form which includes a statement regarding residency outside of Virginia.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 7 children present (14pts) with Provider and 1 Assistant. When the Licensing Inspector arrived, a music program was in progress. Children were observed singing and observing the musician as they played the violin and guitar. Children took turns strumming the guitar at the end of the program. A nice transition from this activity to organized activities which included gross and fine motor skills. The programming area is decorated with artwork completed by the children. Fall trees with leaves were painted by the children and on display. Handwashing procedures observed prior to snack. Snack today was milk, which the children poured individually for themselves and mandarin oranges. A total of 6 children's records and all staff and household member files were reviewed. After snack the children transitioned to outside play. Positive interactions were observed between staff and children. Thank you for your cooperation today. Standard #: 22VAC40-191-40-D-1-c
Description: Based on staff and household record review, repeat background checks every 3 years was not obtained. Evidence: 1 out of 2 staff records did not obtain a repeat criminal history record report every 3 years. Staff #2's last criminal record check is dated 9/24/14. Household #1 did not obtain a repeat criminal history record every 3 years. Household #1's last criminal history record check is dated 10/06/14.

Plan of Correction: Provider is aware of the upcoming national fingerprint requirement for all staff and household members and therefore did not request repeat background checks from Virginia. Provider will request background checks for Staff #2 and Household #1 and will forward requests to the licensing agency within 10 days.
