An exit meeting was completed by the licensing inspector with the provider to review the inspection findings.
A Subsidy Health and Safety inspection was conducted on Tuesday, November 7, 2017 from approximately 11:40 am to 1:00 pm to review supplemental health and safety requirements. Upon the licensing inspector's arrival, the children were observed at the family day home in the designated area participating in a variety of school activities to include relay racing for physical education and interacting with peers. The licensing inspector received assistance from the provider throughout the inspection. During this inspection, there were 5 children totaling 9 points being cared for by the caregiver. The children in care were observed by the licensing inspector participating in various activities to include playing with age appropriate toys, watching Despicable Me, playing with a variety of instruments, and listening to interactive musical songs. The licensing inspector also interacted with the caregiver and the children when appropriate. The caregivers were observed providing age appropriate supervision and addressing children's overall needs. On 10/31/2017 the renewal inspection was initiated in which the bathroom, kitchen, and designated childcare areas were inspected and these areas were reinspected during this inspection as well. The designated areas for childcare were deemed appropriate and equipped with age appropriate toys and materials to meet the ages and stages of development of the children. The inspector reviewed the required postings to include license, prior inspection summary, emergency contact information and menu. The first aid kit and emergency supplies, fire, shelter-in-place, and lockdown drills, thermometers, and emergency preparedness plan were inspected and determined to be in compliance with the licensing standards. The lockdown procedures and emergency preparedness plan were reviewed by the inspection. The emergency preparedness plan was last updated on 07/05/17. There were five children's, the provider's, one provider's assistant, and two household member's records reviewed for compliance of standards. There are currently no medications being administered by providers.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDHOM-000-012-C Description: Based on review of caregivers records, the provider did not ensure that all caregivers completed the Virginia Pre-service Training for child care staff. Evidence: In the assistant caregiver's record, there was not a certificate of completion found for the Virginia Pre-service Training. Interview with the provider revealed that the assistant caregiver has not yet completed the required training at this time. Plan of Correction: The caregiver will complete the Virginia Preservice Training and a copy of the certificate of completion will be placed in the record.
Standard #: VENDHOM-000-012-F Description: Based on review of caregiver's record, the provider did not ensure all caregivers have CPR and First Aid certification within 30 days of employment or 90 days from subsidy vendor approval.
Evidence: In the caregiver's assistant, there was no certificate of completion for CPR and First Aid found in the record. Interview with the provider revealed the assistant recently complete the CPR and First Aid training but the certificate of completion has not been obtained at this time.
Plan of Correction: A copy of the CPR and First Aid certificate of completions will be retrieved for caregivers record and a copy will be included in the record.
