Technical Assistance:
Due to the licensee?s non-compliance with the Mandated Federal Crib Standards, the licensee must come into compliance and submit appropriate documentation to the licensing inspector within 30 days of receipt of the violation notice. The licensing inspector and the licensee discussed that any non compliant crib(s) in the home can only be used for the licensee?s own children or children that reside in the licensed family day home. The licensee has identified to the licensing inspector all cribs that will be used for children enrolled in the family day home.

An unannounced monitoring inspection was conducted this date. A total of 4 children (12pts) with Provider present today. Children were observed playing with large legos, looking at books and transitioning to lunch. Observation of the physical plant was made, one violation found regarding a crib on site. All four children's records, Provider and two household member records were reviewed and found complete. Thank you for your cooperation today. Please email me at:

Standard #: 22VAC40-111-240-A
Description: Based on observation of the home and telephone call to the manufacturer, one crib was determined unsafe. Evidence: The Foundations crib with a manufactured date of June 2009. This crib is not compliant with the Consumer Product Safety Commission requirements.

Plan of Correction: This crib will be removed immediately and replaced with a newly purchased crib. The other crib which is compliant will be used in the meantime.

Standard #: 22VAC40-111-320-B
Description: Based on observation of the home, one outlet was found without a protective cover. Evidence: The outlet located behind the crib closest to the outside door was missing a protective cover.

Plan of Correction: I will place an outlet cover in this outlet.
