Comments:
An unannounced renewal inspection was conducted today with the provider from 11:30 am - 1:45 pm. Nine children, ages three months to four years old were present with the provider and one assistant for a total of 29 points. The children were observed during rest time and free play. Seven children's files, 2 two provider's files and two household member's files were reviewed.

Violations:
Standard #: 22VAC40-111-900-B
Description: Based on observation, children were allowed to drink while lying down. Evidence: An 11 month old infant was resting in a crib with a bottle.

Plan of Correction: The bottle was removed during the inspection and bottles will not be placed with sleeping children.

Standard #: 22VAC40-111-180-A
Description: Based on a review of household member's files, not all household members had a current Tuberculosis Screening. Evidence: Household member #2 turned 18 years old on 2/4/17 and does not have documentation of TB screening.

Plan of Correction: All household members will obtain a TB screening within the next two weeks.

Standard #: 22VAC40-111-280
Description: Based on observation, not all cleaning and disinfectants were kept in a locked area. Evidence: The bathroom cabinet had a lock, however the lock was not working at the time of the inspection and Windex, Chlorox and sanitizer were observed in the cabinet.

Plan of Correction: The lock on the cabinet will be replaced today.

Standard #: 22VAC40-111-320-A
Description: Based on observation, not all electrical appliances were unplugged. Evidence: In the upstairs kitchen, the toaster oven, Kitchen Aid Mixer, and blender were plugged in when not in use by the provider.

Plan of Correction: The appliances were unplugged during the inspection and will remain unplugged when not in use.

Standard #: 22VAC40-191-40-C-1-a
Description: Based on a review of household member's records, not all household members have background checks. Evidence: Household member 1, age 14, date of birth 1/3/2003, does not have a search of central registry. Household member 2, age 18, date of birth 2/4/1999, does not have a sworn statement or criminal background check.

Plan of Correction: The background checks were sent out on October 30, 2017.
