54.1-3408 Provider must be MAT certified to administer prescription medication.

Comments:
An unannounced monitoring inspection was conducted from 10:00am-11:30am. Upon arrival there were 9 children in care totaling 25 points with 2 caregivers. A sample of 3 caregiver and 4 children's records were reviewed. The licensee does not administer medication at this time. Children were observed working on sounds with a caregiver and being read a book.

Violations:
Standard #: 22VAC40-111-770
Description: Based on observation the licensee failed to ensure there was a working battery-operated radio. Evidence: The licensee had a radio and batteries, however the batteries did not work in the radio.

Plan of Correction: The licensee will purchase new batteries for the radio.

Standard #: 22VAC40-111-120-B
Description: Based on a record review the licensee failed to ensure all required information was kept for each assistant. Evidence: Caregiver #3 did not have the following information in the file: verification of age, job title, date of employment, emergency contact.

Plan of Correction: The licensee will obtain all required information for caregiver #3 and document it in the record.

Standard #: 22VAC40-111-320-A
Description: Based on observation the licensee failed to ensure small electrical appliances were unplugged. Evidence: In the kitchen there were 2 coffee makers that were plugged into the electrical wall outlet.

Plan of Correction: The licensee will ensure the coffee makers are unplugged after use.

Standard #: 22VAC40-111-560-A
Description: Based on observation the licensee failed to ensure direct supervision of each child. Evidence: A 1 year-old child was observed sleeping in a room with the door closed. The child was not checked on from 10:15am-11:00am. The licensee stated there is a monitor, however it was not in use at the time of inspection.

Plan of Correction: The licensee will ensure the monitor is used when children are sleeping in a separate room and 15 minute checks will be done.
