54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was initiated today at 1:30pm. The physical plant, 4 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 8 children, totaling 24 points, under the supervision of 3 caregivers. During the inspection, children were observed resting at nap time, coloring pictures, being fed (infants), and enjoying free play. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served chicken nuggets, fries, mixed berries,yogurt, and milk. Two of the caregivers are currently certified in MAT (medication administration training). Should you have any questions, please contact me at email@example.com.
Standard #: 22VAC40-111-60-A Description: Based on file review, 1 out of 5 children's records has not been updated annually. Evidence 1. Child #4's record was last updated on 10/31/16. Plan of Correction: The Provider will ask the parents to update and sign the child's record.
Standard #: 22VAC40-111-90-C Description: Based on file review, 1 out of 5 children's records do not have documentation of additional immunizations once every six months up to the age of 2 years. Evidence 1. Child #4 is 15 months old. The most recent immunization on file was completed when the child was 7 months old. Plan of Correction: The Provider will ask the parents for the most recent immunization record.
