Technical Assistance:
* Look for e-mails from VDSS with more information regarding the national fingerprinting that is required for all household members over age 18 and assistants.

An unannounced monitoring inspection was conducted with provider and two assistants. There were 12 day care children present, ranging in age from 20 months to four years, totalling 25 points. Another caregiver was visiting with an infant this morning. The children were having free play with age appropriate toys, eating lunch, and preparing for rest time. Lunch served today: chicken nuggets, peas, pears, and milk. Two children's files, two staff files, and two household files were reviewed - please see Supplemental Summary for missing items. The pet vaccinations are current (one is due 3/2018). Provider does not administer medications - Medication Administration Training is required to administer medicine. Standard #: 22VAC40-111-180-A
Description: Based on review of two household files, the provider failed to have documentation of an updated TB/PPD test for each household member. Evidence: the TB test for Household member 2 expired 8/17/2017.

Plan of Correction: Household member 2 will get an updated TB/PPD test.

Standard #: 22VAC40-111-210-A
Description: Based on review of two staff files, the provider failed to have documentation of 16 hours of annual training in a twelve month period. Evidence: Staff A (date of hire 7/26/2015) did not have documentation of sixteen hours of annual training.

Plan of Correction: Staff A will provide documentation of college transcripts from this semester.
