Licensing Inspector conducted an unannounced Monitor Subsidy Inspection on May 18, 2023. There were 58 children ages 2 months to 5 years old present under the direct care of 12 staff. The Licensing Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.058 Description: Based on inspection, the Vendor failed to ensure each building that was built before 2015 and houses a child day programs is equipped with at least one carbon monoxide detector.
EVIDENCE:
Center was not equipped with a carbon monoxide detector.Plan of Correction: Center will purchase and install carbon monoxide detector for each building.
Standard #: 8VAC20-790-670-E Description: Based on inspection & discussion with the Center Director, the Vendor failed to develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.
