Comments:
An unannounced monitoring enforcement watch inspection was conducted today between the hours of 9:10 am through 11:00. There were a total of 5 children (23 points) with 2 caregivers. A total of 3 children's files and 1 staff/household member's files were reviewed. The children were engaged in the following: toy play, and infants were sleeping. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's record, the provider has not obtained parents signed acknowledgement of the receipt of the information required. The provider has not obtained the medical policy number for one new child. Evidence: The record for child #1 is missing a parent signature on the Information for Parents form. The record for child # 2, first day of attendance 9/11/2017 does not have the medical policy number.

Plan of Correction: I will have the parent sign the Parent Information form for child # 1. I will ask the parent for the medical policy number for child # 2.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the parent did not obtain before the child's first day of attendance signed acknowledgement of liability insurance. Evidence: The record for child #1 is missing the signed acknowledgement for liability insurance.

Plan of Correction: I will give the form to the parent to sign.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider has not obtained documentation that the child is adequately immunized. Evidence: Child #3, first day attendance 11/27/17 does not have documentation of immunization record.

Plan of Correction: I will ask the parent for the immunization record.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not install child resistant protective covers on all electrical outlets. Evidence: Upon arrival to the family day home, there were 3 toddlers playing in the child care area. There was one electrical outlet that was not plugged.

Plan of Correction: Within 15 minutes the provider plugged the outlet.

Standard #: 22VAC40-111-530-A
Description: Based on observation and provider interview, cribs used for sleeping did not have a bottom and top cover. Evidence: Upon arrival to the family day home there was one infant asleep in the crib. The crib did not have a bottom sheet or top cover. The provider arrived within 15 minutes with the crib sheets.

Plan of Correction: A crib sheet was placed on the crib.

Standard #: 22VAC40-80-120-E-1
Description: Based on observation and provider interview, the most recent issued license is not posted at each public entrance. Evidence: The most recent provisional license issued September 2, 2017 was not posted. The provider was asked about the license and it was located and immediately posted.

Plan of Correction: The provider posted the license.
