Technical Assistance:
Consultation provided on supervising children and completing background checks.

Comments:
A complaint was received involving inadequate supervision of a child. The investigation was conducted in conjunction with the local department of social services. Interviews, observation and a review of recordkeeping were conducted and the complaint was determined to be valid.

Please submit documentation that you have sent off for all missing background checks within 10 days and then provider the documentation of the completed background checks.

Based on interview and a review of recordkeeping provided by the facility, it was determined that all staff and volunteers functioning as staff were not certified by a physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.
Evidence:
On May 9, 2023, Staff B was present and working with children and did not have documentation on file that they are free from any disability which would prevent them from caring for children under their supervision.

Based on interviews, it was determined that a child was not appropriately supervised to include procedures to ensure their safety.
Evidence:
On 4/26/23 at approximately 2 pm the parent of child A ( 3 years of age) arrived at the center to pick up their child. When they arrived they didn't see the child in the main daycare area, the parent heard their child crying from the office, which is located off of the main daycare space. The door to the office was closed and the parent and staff A found the 3 year old crying behind the door of the office for an undetermined amount of time.

Interviews revealed that on 4/26/23 child A had been placed in timeout on 3 occasions and each time, the child left the timeout location and entered the office area unsupervised, which was where the staff found the child on each of those 3 occasions.
