Comments:
The Licensing Inspector for the Family Day Home of April Caudle, conducted an unannounced Renewal Inspection on Monday, December 11, 2017. There were six (6) children present under the direct care and guidance of two (2) caregivers. Children present were of various ages from 9 months -4 years old; adult-to-child ratios were met. Children & Staff/Household records were reviewed. Provider continues to participate in the USDA Child Care Food Program, does not provide transportation and does not administer medication. Violations found during the previous inspection were reviewed and have been corrected. An exit interview including review of the Risk Assessment & areas of non compliance found during today?s inspection were reviewed with the Provider on December 11, 2017 PM. Inspection concluded at 4:15 PM.

Standard #: 22VAC40-111-210-A
Description: Based on record review, the Licensee failed to ensure each caregiver completes 16 clock hours of training annually. EVIDENCE: Staff #3 did not complete 16 hours of annual training. Staff #3 has completed 11 hours of annual training.

Plan of Correction: Staff will complete annual training.

Standard #: 22VAC40-111-320-G
Description: Based on inspection & discussion with the Provider, the Licensee failed to have fireplace and associated chimneys inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. EVIDENCE: Fireplace and associated chimney locate on the ground floor has not been inspected annually. Most recent inspection completed on 10/30/2014.

Plan of Correction: Provider will have fireplace and chimney inspected.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on record review & discussion with the Provider, the Licensee failed to update Sworn Disclosure Background Check . EVIDENCE: Licensing Inspector reviewed three (3) staff records. The Provider & all staff members did not complete/update Sworn Disclosure; which requires disclosure of previous out-of-state residency.

Plan of Correction: Will have everyone complete new
