- Technical Assistance:
Reviewed all current and upcoming changes to background check procedures. All future information regarding background check procedures will only be communicated via email. Only the child's parent shall sign/date the annual review of the child's registration form and contact information. The provider shall empty the home of much of the unnecessary items that are in the home and in the basement child care area.
An unannounced monitoring inspection was conducted today. The provider and one temporary assistant caregiver were caring for 9 children, totaling 21 points. Children's records, staff records, staff training records, emergency preparedness items, and the physical plant were inspected today. The provider reported to the Inspector that no children's medications were on site today. The children were observed playing with toys, listening to stories in Spanish, and interacting with both caregivers. The children were to be served chicken with rice and mixed vegetables, apples, pears, and milk for lunch today. This inspection was completed between approximately 9:20am and 11:45 am. Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, it was determined that one record was not complete. Evidence: The record for caregiver B does not include date of hire and emergency contact information. Plan of Correction: I will have her fill out the form when she returns to work on Monday.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #1 does not include the parent's signature indicating authorization for the provider to obtain emergency medical care if needed. Plan of Correction: I will ask the parent to sign that section of the registration form today.
Standard #: 22VAC40-111-70-A Description: Based on a review of children's records, it was determined that two records do not include all required written information provided to parents. Evidence: Child record #1 and #5 do not include a "Medication Policy" document signed by the parent. Plan of Correction: I will ask both parents to sign the form today.
Standard #: 22VAC40-111-90-A Description: Based on a review of children's health records, it was determined that one record does not include an immunization record. Evidence: Child record #1 does not include the child's immunization record. This child began enrollment on 11/127/17. Plan of Correction: I will remind the parent to bring the shot record.
Standard #: 22VAC40-111-340-C Description: Based on observations, it was determined that the children's bathroom was not equipped as necessary. Evidence: The children's bathroom was not supplied with paper towels. Plan of Correction: Paper towels were replaced immediately during the inspection.
