Technical Assistance:
An exit meeting was completed by the licensing inspector with the caregiver to review today's inspection findings.

The licensing inspector conducted an unannounced renewal inspection on Thursday, January 11, 2018 from approximately 9:50 am until 11:30 am to determine the caregiver's compliance with licensing standards. During this inspection, there were 10 children totaling 35 points being cared for by three caregivers at the family day home. The children were observed in the designated area childcare listening to interactive music, interacting with their peers and caregivers, engaged in play, sitting in age appropriate equipment and asleep in their crib. A formal observation was completed by the licensing inspector of the children watching an interactive program and dancing to the song, Wheels on the Bus. The licensing inspector completed observation of the caregiver preparing for lunch. The children's lunch today consisted of chicken noddles, green beans, mixed fruit, and milk. The designated areas for childcare were deemed appropriate and equipped with age appropriate toys and materials to meet the needs of the children. During the inspection, the licensing inspector interacted with staff and children when appropriate. The bathroom, kitchen, and designated childcare areas were all inspected during today's inspection. The inspector observed the required postings to include license, prior inspection summary, evacuation mapping. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills were inspected. During this inspection, four children's, the provider's, the provider's assistant, and a household member's records were reviewed for compliance of standards. There are currently no medications being administered by the caregivers. The licensing inspector completed some but not all licensing standards during this inspection.

The violations from the previous inspection were checked for corrections. The Department must receive a complete renewal application and fee prior to expiration of the license on 01/23/18 or the Department cannot renew the license. Description: Based on review of caregiver's records, the provider did not ensure TB tests were updated every two years.
Evidence: In the assistant caregiver's record, the last dated TB test was 10/17/15. Interview with the caregiver revealed that a TB screening had been completed but an updated copy was not obtained by the licensee for the record. TB tests are required to be updated every 2 years.

Plan of Correction: The caregiver will ensure a repeat TB screening is completed or a copy of the assistant caregiver's updated TB screening record is obtained by the physician and a copy is included in the record. The provider will ensure each caregiver's TB screening is completed every two years.
