54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted today from approximately 10:30am-1:10pm. There were 7 children (19 points) directly supervised by 2 caregivers. A sample of 4 children?s, 2 caregiver's, and 1 household member's records were reviewed. The children were observed dancing, playing with toys, and interacting with the provider and assistant. 1 new caregiver has been hired since the last inspection. Standard #: 22VAC40-111-830-B Description: Based on record review and interview, shelter-in-place procedures were not practiced a minimum of twice a year. Evidence: There were not shelter-in-place drills practiced during the year of 2017. The provider stated that she did not know how to conduct the drill. Plan of Correction: I will continue drills this year.
Standard #: 22VAC40-111-50-A Description: Based on record review, attendance for each child was not kept each day. Evidence: The last record of attendance was taken on 1/4/2018. Plan of Correction: Corrected during inspection
Standard #: 22VAC40-111-60-A Description: Based on record review, children's records were incomplete. Evidence: 2 of 4 children's records reviewed were incomplete. 1) Child A was missing 1 of 2 emergency contacts. 2) Child B was missing 2 emergency contacts, mother's work number, father's employer, and proof of identity. Evidence: Caregiver A, hired 10/17/2017 had not been oriented by provider at the time of inspection. Plan of Correction: I will put her through orientation.
