Today we discussed outside play equipment, menu substitutions and documentation of first day of attendance.
An unannounced monitoring inspection was conducted from 2:15pm through 4:30pm. There were six children in care ranging in age from 14months through 3.5 years, including one resident child. There was a total of 15 points present with the provider. Children's records were reviewed. Children were observed in play activities inside and outside, rest room routines, hand washing and snack time. Required postings were reviewed. One violation regarding proof of identity was cited.
Standard #: 22VAC40-111-60-B Description: Based on record review and interview the provider failed to ensure that each child's record contains all required information. Evidence: The records for child 1 and 2 do not contain clear documentation that an acceptable form of proof of identity was viewed. 1 - The enrollment form for child 1 identifies the number written on the form as the SSN. 2 - The provider stated that SSN stands for social security number. 3 - The enrollment form for child 2 identifies the number written on the form as both the SSN and the Birth Cert. Plan of Correction: The provider will ensure that the information documented is the required form, a birth certificate for example.
