54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today from approximately 10:50am-12:00pm. There were 10 children (28 points) directly supervised by 2 caregivers. A sample of 5 children?s records, 1caregiver's record and 1 household member's record was reviewed. The children were observed watching television and playing with toys. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-800-B Description: Based on observation, the emergency plan was not reviewed annually. Evidence: The emergency plan was last reviewed 1/6/2017. Plan of Correction: Completed during inspection.
Standard #: 22VAC40-111-830-B Description: Based on record review, shelter-in-place drills were not practiced a minimum of twice per year. Evidence: For the year of 2017 there were no shelter-in-place drills practiced. Plan of Correction: I will conduct two drills this year.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 5 of 5 records reviewed were incomplete. 1) Child A was missing an annual update. File was last updated 8/10/2015. 2) Child B was missing an annual update. File was last updated 11/27/2016. 3) Child C was missing an annual update. File was last updated 9/6/ 2016. 4) Child D was missing the original signature of the parent and people authorized to pick up the child. 5) Child E was missing fathers work phone number and 1 of 2 emergency contacts. Plan of Correction: Parents will update files today.
Standard #: 22VAC40-111-180-A Description: Based on record review, a tuberculosis screening was not repeated after the two years. Evidence: 1) Household Member A's tuberculosis test was last conducted 8/8/2015 and expired 8/8/2017. Plan of Correction: He will get it done tomorrow.
