An unannounced renewal inspection was initiated today at 11:45am. The physical plant, 3 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 4 children, totaling 8 points, under the supervision of the Provider. Today, the children played outside, watched an educational video, prepared for and ate their lunch. The interactions between the children and Provider was seen to be positive and constructive in nature. Today, the children were served chicken, rice, mixed vegetables, chapati (flat bread) and milk. The Provider is not currently certified in MAT (medication administration training) and there are no children with any medication needs during program hours. Description: Based on observation, the Provider did not keep a written record of children in attendance each day. Evidence 1. Upon entering the FDH, the attendance sheet for today was blank.

Plan of Correction: This was completed during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 3 out of 4 children's records do not have all required information. Evidence 1. Children #1, #3 and #4 do not have the start date that they began receiving care at the FDH. 2. Child #4 does not have the name, address and telephone number of each custodial parent's place of employment. 3. Child #1 does not have the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached

Plan of Correction: The Provider will record the start dates and ask the parents for the other information.
