Comments:
An unannounced monitoring inspection was conducted today. The provider was not present during this inspection. The assistant caregiver was caring for three children, totaling 7 points. One of the children in care was the provider's own child. Children's records, caregiver records, background checks, caregiver training records, the physical plant, children's nutrition, and emergency preparedness items were inspected today. The children were observed playing with toys, practicing the alphabet, listening to music, and singing songs. The children were to be served pizza, peaches, bananas, and milk for lunch later today. Standard #: 22VAC40-111-50-A
Description: Based on a review of administrative documents, it was determined that the provider has not kept a written record of children's attendance daily. Evidence: The provider has not documented the written record of children's daily attendance since February 12, 2018.

Plan of Correction: The attendance will be recorded daily.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #1 does not include the father's work contact information and documentation showing that the provider has viewed the child's original proof of identification. 2. Child record #2 does not include documentation showing that the provider has viewed the child's original proof of identification.

Plan of Correction: Both files will be updated.

Standard #: 22VAC40-111-70-A
Description: Based on a review of children's records, it was determined that two parents have not been given all written program information as required. Evidence: 1. Child record #1 does not include a completed "Information for Parents" document. The parent has signed this form, but the provider has not recorded any of the required information on the form. This parent has also not been provided the provider's written medication policy. 2. Child record #1 and #2 do not include a completed "Provisions of the Emergency Plan" document. Each parent has signed this form, but the provider has not recorded the required information to notify the parents of the off-site emergency relocation address.

Plan of Correction: All missing information will be completed.

Standard #: 22VAC40-111-320-B
Description: Based on observations, it was determined that three electrical outlets were not covered. Evidence: In the infant nap room, three electrical outlets on the lower portion of the wall were not covered.

Plan of Correction: The outlets will be covered today.
