Discussed the fingerprint requirements for all household members over 18 years of age and staff. Provider given contact information for Office of Background Investigations. You are reminded that all fingerprinting is required to be completed by September 2018.
An unannounced monitoring inspection was conducted this date. A total of 7 children present (22pts) with Provider and 1 assistant. When the inspector arrived, children were finishing lunch which was spaghetti today. Hand washing procedures observed after lunch. A nice transition was observed from lunch to quiet book time to nap time. The infant room was observed and found clean and well maintained. Staff observed holding children when being bottle fed. A total of 6 children's records and all staff and household member records were reviewed. Previous violations found at the July inspection were reviewed and corrective actions were verified. An improvement found regarding recordation and record organization. No medications at this time and no food allergies identified. Thank you for your cooperation today. Standard #: 22VAC40-111-100-B Description: Based on child record review, one physical exam was not conducted within the required time frame. Evidence: Child #2 is 18 months old and began care on 2/20/18. The date of the physical exam is 9/16/16. The physical exam must be completed within the past 3 months of the enrollment date. Plan of Correction: I will request a new physical exam from the parent today.
Standard #: 22VAC40-191-40-D-5-a Description: Based on household record review, two child protective services central registry checks were not completed. Evidence: Two household members ( # 2 and # 3) are over the age of 14 years. A central registry check was not obtained for these two household members. Plan of Correction: I will request a central registry check on both of these household members.
