- Technical Assistance:
Discussed required documentation for children's records to include ensuring physical examinations and immunizations being signed by the physician, physician's designee or health department official. Medication is to be kept locked. In order to administer medication, the individual has to be Medication Administration Trained (MAT) certified. Please see individuals approved to conduct that training on the department's website at www.dss.virginia.gov.
An announced initial inspection was conducted on this date. Provider was present with four children, totalling eight points. Children were observed during nap time. Provider record was reviewed. Space and equipment were observed. Emergency preparedness plan and required postings were reviewed.
Standard #: 22VAC40-111-730-D Description: Based on observation, provider did not ensure medication is kept locked. Evidence: On February 28, 2018, Albuterol medication was observed in a child's backpack in a open cubby in the main childcare room. Plan of Correction: Provider will give it back to parent today.
Standard #: 22VAC40-111-770 Description: Based on interview, provider did not ensure there is a working portable battery-operated weather band radio. Evidence: On February 28, 2018, provider stated she does not have a weather-band radio. Plan of Correction: Provider will buy a weather-band radio.
Standard #: 22VAC40-111-920 Description: Based on observation, provider did not ensure when meals or snacks are served that there is a current one-week menu posted. Evidence: On February 28, 2018, the menu posted shows current day, February 28, foods and snacks served. Plan of Correction: Provider will print out a menu for the week and post it.
Standard #: 22VAC40-111-330-A Description: Based on observation, provider did not ensure there is a landline telephone, excluding a cordless or cell phone, available that is operable and accessible during the family day home home's hours of operation. Evidence: On February 28, 2018, the landline available and operable is a cordless phone. Plan of Correction: Provider will bring in a landline from the shed that is not a cordless phone.
Standard #: 22VAC40-111-520-C Description: Based on review and interview, provider did not ensure a crib meets current federal crib requirements. Evidence: On February 28, 2018, one crib was present in the childcare room. Provider called manufacturer during inspection and it was confirmed the crib does not meet current federal crib requirements. Plan of Correction: Provider will remove the crib completely from the house. Provider will purchase a new crib.
Standard #: 22VAC40-111-530-A Description: Based on observation, provider did not ensure 4 of 4 children observed during nap time have a top and bottom linen. Evidence: On February 28, 2018, children were observed napping on mats and they do not have a top cover. Plan of Correction: Provider will label some blankets she already has and use them.
Standard #: 22VAC40-111-690-D Description: Based on observation, provider did not ensure the diapering surface is non-absorbent and washable. Evidence: On February 28, 2018, the diapering table is wooden and there is not a non-absorbent pad. Plan of Correction: Provider will buy a diaper pad.
