Today an unannounced renewal visit was conducted with the Provider. This visit took place in the morning between 12:30-2pm there were 6 children with the provider and 1 assistant for a total of 18 points within the supervision guidelines. The children were observed having lunch, afternoon outdoor free play, and nap time. Lunch consisted of vegetable soup, fruit and milk. Positive interactions were observed during this inspection. Standard #: 22VAC40-111-920
Description: Based on review of the weekly menu, the menu posted for did not reflect the substitution of what the children were served. Evidence: The menu posted for today stated white rice, lentils, tomatoes and fruit. The children were observed eating a chicken, vegetable soup.

Plan of Correction: Provider stated that she plans to serve the children lentils and rice for dinner. But will make sure that what is on the menu is what is being served.

Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were observed labeled however they did not include a date. Evidence: 2 bottles were labeled however they did not include a date.

Plan of Correction: Provider stated that she will add the date with tape.

Standard #: 22VAC40-111-60-B
Description: Based on review of seven children?s records, the provider did not maintain complete children?s records. Evidence: Child #1's record was missing medical insurance policy number.

Plan of Correction: Provider stated that she will ask parent today at pickup.

Standard #: 22VAC40-111-230-A
Description: Based on review of provider records, one caregiver record did not have documentation that staff received orientation training that includes job responsibilities, policies/procedures, playground safety procedures, child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibility. Evidence: Caregiver #1 did not have documentation of orientation training.

Plan of Correction: Provider will have her complete it today.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially hazardous substances such as cleaning agents and pesticides were stored in an area that was not inaccessible to children. Evidence: Can containing paint were found in the backyard accessible to children while they were playing.

Plan of Correction: Provider stated that she will remove them and place in the trash today.
