Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and board officers must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. Based on the information on file, the Family Day Home (FDH) will be able to start their fingerprint checks on August 1, 2018. If you did not receive your Facility ID number, your Field Print code numbers or you have not establish a point of contact, then please contact the Office of Background Investigations (OBI) at 1 (804) 726-7884.
The following inspection took place between the hours of 2:30 pm through 4:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced renewal inspection there were a total of 9 children (7 points) with 1 caregiver. A total of 9 children?s files, 1 staff file, and 3 household member?s files were reviewed. The children were engaged in the following: drop off, snack, homework time, story time, learning, reading, and other organized activities. If you should have any future questions, please call. Standard #: 22VAC40-111-100-A Description: Based on review, 3 of 9 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (03/12/2018), the following children are without documentation of a physical examination: Child #2, Child #7, and Child #8. 2. Their start dates are: 08/24/2017 (Child #2), 11/27/2017 (Child #7), and 11/27/2017 (Child #8). Plan of Correction: Parents shall be contacted in order to obtain this documentation. If possible, a copy might be obtained through each child's school (with the permission of their parent).
